Palliative care is a type of supportive care that can be provided concurrently with curative or life-prolonging medical care. All patients, regardless of their life expectancy are eligible for palliative care. Hospice care necessitates a life-expectancy of 6-months or less and is given to those nearing the end of lives exclusively.

It is never too early for palliative care. We care for patients and families in all different stages of illness. Our goal is to help optimize quality of life regardless of where you are in your disease trajectory.

No. We care for patients with a range of diagnoses including but not limited to: Heart Disease, Dementia, Liver Disease, Kidney Disease, Parkinson’s Disease, Stroke, ALS, and more.

We unfortunately do not offer Home Health Aid or live-in caregiver support. We can however help you to obtain these services by exploring what you/your loved one qualifies for through insurance benefit or through private hire agencies.

Yes. It is your choice whether you would like our team to co-manage and provide care in collaboration with your existing providers or whether you would like our physician to replace your primary care provider to offer in-home PCP services. We do not replace your existing specialty providers (i.e. oncologists, cardiologists, pulmonologists, etc.) but will continue to work alongside of them to ensure you have the most comprehensive care possible.

Yes, you can request visits not included in the monthly plan for an additional fee.

Holistic care is the provision of care considering physical, psychological, emotional, and spiritual needs. To put it simpler, it is “whole person care”. We evaluate our patients as human beings first. We also think outside of the box in our approach to care, seeking to minimize interventions that may cause unwanted side effects.
