FOUNDER/GERIATRIC AND PALLIATIVE CARE NURSE PRACTITIONER

Rachel Chiulli is a board-certified Adult Gerontology Nurse Practitioner with experience in Emergency Medicine, Hematology/Oncology, Pain Management, Palliative Care, and Hospice Care. After graduating from Columbia University as a Nurse Practitioner, she was Fellowship Trained in Pain and Palliative Care at Memorial Sloan Kettering Cancer Center. She then went on to serve as an inpatient Nurse Practitioner on the inpatient Palliative Medicine Service at Mount Sinai Hospital in NYC.

She most recently developed the Mount Sinai Palliative Care at Home Program. Rachel is dedicated to caring for those with serious illness, combining Eastern and Western Medicine techniques, to ensure her patients receive lowest-risk/highest yield interventions that will optimize their quality of life.
